Patient died after systemic and individual failures: jury highly critical

The jury in the inquest into the death of Jason Walker has returned a narrative verdict identifying 4 systemic failures and 14 individual failures that caused or materially contributed to his death. After 11 days of evidence, the jury found that Jason, a patient in an acute admissions mental heath ward, could have taken his life any time between 21.30 and 07.30 without being detected by staff because 10 minute observations were not being performed sufficiently.
