Butler Inquiry report finds NHS staff "let down" killer and victim

The independent inquiry into the care and treatment of Glaister Earl Butler, chaired by Robert Francis QC, found that both Mr Butler, a paranoid schizophrenic, and the man he killed, Detective Constable Michael Swindells, were let down by NHS care staff.

 

The report found that Mr Butler had received only "superficial contact" from the mental health team responsible for monitoring him and that medication was often posted through his letterbox. Mr Butler had not taken his medication for more than a year when the incident took place.
